My brother in law's wife is a nurse at the VA, she's dealing with it every day.  My uncle and father and doctors with doctor friends they stay in touch with.  This is the part I really don't understand.  there are tons of republican doctors, nurse, hospital admins etc.  They have families and friends who are republicans too.

 

Where is the mass of pro Trump doctors and nurses flat out calling this a hoax?  Because it is two completely different things to say we never should have shut down the way we did and saying the whole things isn't happening and that 100K is not a solid estimate of the deaths.

CORONA DISEASE 2019 TEST RESULTS AFTER CLINICAL RECOVERY AND HOSPITAL DISCHARGE AMONG PATIENTS IN CHINA

It is known that SARS-CoV-2 nucleic acid has been demonstrated in patients who have clinically recovered for up to 20 to 30 days.  It is felt that this material does not represent infectious virus despite the positive RT-PCR test results but rather fragments of RNA material.

Two patients were discharged who had previously been diagnosed with, and hospitalized for Covid-19, were readmitted to a hospital for symptoms consistent with Covid-19 and found to have test results positive for Covid-19.

As a result of these two patients, nasopharyngeal swabs and anal swabs from 58 patients who had been hospitalized for Covid-19 and discharged prior to 2/27/20 in Loudi China were obtained.  

Sixty patients with a median age of 46.5, 43.3% were females were in the study group.  A total of 10 patients (16.7%) had positive tests.  Five patients (8%) had positive nasopharyngeal swabs, six patients (10%) had positive anal swabs and one patient was positive for both.  Only patients one and two had mild symptoms of cough and both were over 70 years of age with multiple underlying medical conditions.  These patients were tested anywhere from four to twenty four days after discharge.

One of these patients donated convalescent plasma.  Nine of the staff who collected the plasma did not have adequate PPE but al nine staff were developed for 60 days with RT-PCR and symptom screening and all remained well.  Although virus was obtained for culture none of the cultures supported viral growth.

Until more is known, it is probably prudent to wear masks when around patients who have recently been discharged from the hospital with Covid-19.

The clinical arena has become less hectic,  We no longer have the code teams that we had only a few weeks ago.  Many of the units that were repurposed have returned to their normal function.  We have created a Covid preoperative area and postoperative area which has improved the clinical experience with the patient and efficiency of operation.  Before the creation of this unit, Covid positive patients were transported directly into the operating room from their units.  Our reviews with the patients were cursory at best.  The operating room environment is not usually associated with the relaxed tempo that we try and achieve with preoperative visits where we sit down for a few minutes and try and gain the confidence of a patient who is hoping that we will wake them up.  Having the Covid positive postoperative area allows us to take the Covid positive patients to a recovery area rather than recovering them in the operating room which is an extremely high resource area, and very inefficient.

All patients coming to the operating room are tested for Covid-19 within two or three days before the operation, some are tested the day before or even the day of.  We are aware that the tests done before the day of operation are window dressing and do not guarantee that the patients we are treating are not infectious.  Therefore, unless tested the day of, we assume that all the patients we are treating are potentially infectious and proceed accordingly.

Things are much better.

Contact investigations (tracking and tracing) have been a vital public health strategy in controlling contagious diseases.  The large numbers of Covid-19 infections pose major challenges to contact investigations.  The need to make hundred or even thousands of phone calls in a short period of time is very labor intensive.

Strategies in China, Hong Kong, South Korea and Taiwan have supplemented traditional manual approaches with digital surveillance through smartphone applications.  In these countries these digital application for the most part have been engineered toward tracking compliance with quarantines.  At least one country has used a digital application to assess the risk of disease, through proximity to known Covid-19 positive patients to enhance free travel.

The United States has not utilized digital surveillance as a tool, but Google, Apple, MIT and two European Consortia are developing Bluetooth smartphone technologies to enable rapid identification of users that have had a close exposure to individuals with medically diagnosed Covid-19.

Traditional tracing begins with reporting to the Department of Health, which then assesses risk, questions medically diagnosed persons with Covid-19 for their known contacts of potential exposure all by telephone.  Patients are not obligated to disclose contacts and health officials do not inform contacts of the identification of the index patient.

Digital tracing can rapidly notify users if they have been in close proximity with a person with Covid-19.  Digital tracking can detect proximity but not geographic location.  Downloading the application is voluntary.  Users would have the ability to turn on or off location services. This technique relies on an anonymous signals called chirps sent back and forth between phones equipped with the application.  Chirps contain no identifying information on individuals.  Covid-19 diagnosed patients are not revealed except to the Department of Health.

Most countries, such as China, South Korea and Taiwan use a centralized approach to enforce quarantine.  These successful efforts relied on well resourced and rapidly deployed manual contact tracing.  The success of these programs importantly relies on the willingness of the vast majority to abide by quarantine restrictions for the good of society, enabling Public Health Officials to easily confront the few who choose not to  abide.   I do not foresee our society subjugating itself, even in these times of crisis.  If hydroxychloroquine had been mandated, would it have been so popular?  After billions of dollars have been spent on research and individuals have volunteered to be infected with a potentially incurable virus, what percentage of our free spirited public will be willing to receive the vaccine?

This technology is certainly interesting but seems like it is in a development phase and may be useful for the next pandemic.  Only 20% of the citizens of Singapore volunteered to upload the applications.
