There's no guarantee that subsequent waves will happen. There's no guarantee even if a second "wave" doesn't happen this won't come back next season. We just don't know. But instead of recognizing that all predictions (at best) are based on somewhat limited spread models and historical pandemic data and debating with nuance the need to factor both most-likely and worst-case predictions, as well as the need to not economically devastate the country while recognizing the flaws of the models and unknown nature of the disease, we've managed to turn it all into a political issue. It's frustrating AF from a public health perspective. I hate everyone. #nolivesmatter

JAMA Pediatrics May 11 Initial information coming out of China suggested that infants and children infrequently experienced severe disease from the Covid-19 virus, compared with adults. Of some 72,319 Chineses citizens diagnosed with Covid-19, 1.3% were younger than 20. In a subsequent report of 171 Chinese children younger than 16 who were hospitalized, three were admitted to an intensive care setting and one died. On April 6, 2020 the CDC reported that 1.7% of the diagnosed Covid-19 patients in the United States were children. At the time of this report there were about 152,000 diagnosed cases of Covid-19 in the United States. Of these 2,572 cases were pediatric cases, 15 were admitted to Pediatric Intensive Care Units and three patients died. Extrapolated to present day numbers that would represent 29,590 cases with 172 Pediatric Intensive Care Unit admissions and 34 deaths. In March a new voluntary International Covid-19 Pediatric Intensive Care Unit Collaborative was formed, including some 300 Pediatric Intensivists and Infectious Disease specialists fromm 100 of the largest Pediatric Hospitals on six Continents. This group has been meeting virtually twice weekly. There are 50 North American Pediatric Hospitals involved in this collaborative effort. This report focuses on a three week interval from March 14, 2020, to April 3, 2020 with an additional 10 day observation period. The charts of Pediatric Intensive Care Unit Admission from 46 Pediatric Hospitals were reviewed. Of these 46 hospitals, 30 did not have a single Pediatric Intensive Care Covid-19 admission. There were 14 hospitals all in the United States that were able to submit data.. There were 48 admissions to the Pediatric Intensive Care Units of patients less than 21 in these 14 hospitals. Interestingly, almost 50% of the admissions came Children National Hospital in Washington, DC and Children's Hospital Montefiore in New York. Most of the patients presented with respiratory symptoms, and three with Diabetic Ketoacidosis ( a severe complication of diabetes when blood sugars rise to very high levels) Over 50% of the patients had at least one comorbidity. On admission 69% were critically ill, requiring drug therapy to support the blood pressure, 81% required some form of respiratory support and 38% required mechanical ventilation. Two of these patients died and three are still on mechanical ventilation at the end of the observation period. The patients who died, 12 and 17 years old, both had pre-existing comorbidities. In this series, obesity was noted to be a significant comorbidity especially in older children. In this report ,Pediatric Intensive Care Unit Mortality was less than 5% as compared to Adult Intensive Care Unit mortality for Covid-19 which approaches 50%. In the CDC report mortality was 20% for the patients admitted to a Pediatric Intensive Care Unit. We again see that obesity is a significant risk factor for severe disease. If High Schools and Colleges in fact resume football in the fall without daily testing, there could be significant ramifications for the lineman who are typically very large people. Numbers: This week I have looked at confirmed cases from 5/24/20 to 5/30/20 and compared them to the previous weeks numbers in Alabama, California, Connecticut, Florida, GEorgia, Illinois, Iowa, Louisiana, Maryland, Michigan, Mississippi, New Jersey, New York, Ohio, Pennsylvania, Texas and Wisconsin. Last week there were nine states that had more cases than the previous week. This week Alabama, California, Florida, Mississippi and South Carolina are again reporting more new cases this week than the previous week. Alabama had the largest weekly increase at 40%. I haven't seen a good study on hydroxychloroquine and COVID, probably because we're only a couple of months in, and studies of malaria drugs is not high on the list of priorities.
