  • Update  Surgery was a success. Mom has been home since this afternoon. Some pain, but good otherwise and they got the entire tumor.  Thanks all for the well wishes and prayers.

I haven’t been in favor of blaming covid response on trump but that drastic vertical movement in the curve is all trump.

I find it interesting that despite the fact that to this point deaths in Florida and elsewhere are dropping, there isn't a lot of messaging on this. my read is that there's an expectation in Florida and elsewhere that the number of COVID-19 related deaths is going to start going up soon, as it lags behind the case rate typically

I don't know if that will actually happen, given that the median age for cases is plummeting, but I think it's a risk DeSantis and others aren't willing to take right now. 

All these places should be blowing up on deaths right around July 4.

Yup, that's exactly what I said back in April many times.  The first wave is hard to actually pin deaths on Trump (although he definitely carries some blame).  But he is absolutely going to own this second wave in hot summer Trump country that has no business blowing up like this.

Instead of pictures of crowded bars and beaches, it might be more appropriate to produce the portraits of the Political leaders of these locales, who by their ineptness, hypocrisy and failure to serve the public good are most responsible for the spectacle.  That the public good is well served by distancing and wearing masks can no longer be debated.  That increased cases are only a reflection of increased testing lacks credibility as I demonstrated with last Sunday's numbers.

Dismiss case numbers, lest the real issue be clouded by subterfuge.  Mounting Covid-19 hospitalizations and deaths resist concealment.  Shine the spotlight on the Politicians who are responsible for this.  The friends and loved ones of 117,000 dead and mounting, deserve an explanation.  Distancing and wearing masks early would have negated the economic holocaust. To be clear, we did not have to shut down the economy.   The unemployed deserve an explanation.  For those Political leaders who say nothing had to be done, please cast an eye on Brazil.

 

Seven Mayors in Texas were seeking permission from Governor Abbott to mandate masks and he dawdled, and has refused to allow enforceable mandates for wearing masks.   He boasted of the bed capacity in his state, but it is the sick and dying filling those beds in increasing numbers that I distress over. He refers to nursing home patients and prisoners as if they are not human.

To increase compliance and soothe the intellect of the freedom minded, bonuses could be paid to induce increased compliance with distancing and wearing masks. 

The final arbiter is the voter.  It will be interesting to see how many of these freewheeling, free spirited, politicians remain in office after November.  Where are the Ms. Ardens when we need them?

CONVERSATIONS WITH DR. BACHNER, EDITOR

Interview with Dr. Carlos del Rio

Professor at the Emory Vaccine Center

June 15, 2020

This was a 35 minute interview covering a variety of interesting and practical subjects.

Incubation period - the time from exposure until the time of symptoms (in those who develop symptoms)  Dr. Del Rios believes that almost 100% of the incubation periods fall in the 2 to 14 day range.  Defining the incubation period is made more difficult because probably 50% of the population have no symptoms or symptoms that are so mild as not to be recognized.  These asymptomatic people do develop viral loads, shed virus and are infectious.  This is unlike Ebola or Smallpox where disease was not transmitted until after symptoms developed.

Infectious - most people develop enough virus to begin shedding about 2 to 3 days before symptoms develop and are infectious at that time.  The disease is not transmitted by just having passing contact with an infected person unless they cough or sneeze in your face.  Rather 10 to 15 minutes of unprotected contact within six feet especially if indoors is necessary for transmission.

The fact that asymptomatic people are infectious and that the mode of spread is through respiratory  transmission, is why masks need to be worn.  Dr. Del Rios spent considerable time discussing the fact that people do not seem to understand the imperative of wearing masks and that they appear to believe that they can return to pre Covid-19 behavior.  Masks are easy to wear, have no side effects and also help to signal to others that "something" is up.  He equates them to a successful vaccine.  I certainly do not have the credentials of Dr. Del Rios, but his focus on the people misses the mark.  Any good leader, be it political (Ms. Arden),  generals (Patton), coaches (Lombardi) have/had the common ability to sell their message and get others to buy in.  Times are critical and it is time for serious leaders.  The spot light needs to be on those leaders who are failing to deliver a message that will protect the public.  Name names and make them feel uncomfortable.

These Anti Science governors sit back and proffer hypocrisy as their citizens fall ill and die.  Each of these states has enforceable DUI laws, enforceable speeding limits, and enforceable laws for driving without a license!  Everyone of these states has lost many more citizens to Covid19 in four months than they have to all cause traffic fatalities in 2018.  An enforceable mandate for wearing masks would have prevented this tragedy. Hypocrites!!!  

 

Returning to work after infection - 10 days after a positive test or three days after symptoms have abated, whichever is LONGER.

Dr. Del Rios does not believe that Vitamin D has any role in treating or preventing Covid-19

Dr. Del Rios reviews a Study that I summarized in the 6/14 email out of China examining the use of Convalescent Plasma and concludes that maybe we need to use such therapies earlier in the disease.

He does not believe that a vaccine will be ready before the end of the year and expects that efficacy will be about 30% to 40% in prevention and possibly make the disease less deadly, as opposed to total prevention.  He is worried that politics will interfere with the normal safe development and evaluation of a vaccine and that shortly before the election a vaccine will be touted as the miracle cure when in fact it is not.

In the USA,  85% of the cases occur in individuals under the age of 65 but 85% of the deaths occur in individuals over the age of 65.  Dr. Del Rios hopes that greater efforts will be made to protect the nursing home patients, prisoners, meat packing industry and those living in the crowded inner cities who are most vulnerable.

Schools - in general Dr. Del Rios does not think that children can be sent back to school as if pre Covid-19 conditions existed.  Possibly education in shifts, or a combination of live and online learning.  Also, this should not be a country wide template because there are many communities where the prevalence is so low that little will need be done.

The global problem, specifically looking at Brazil and Mexico.  Both countries have already out stripped their ICU bed capacity.  In Brazil especially, where their brain dead President is unconcerned, they currently have a need for over 12,000 ICU beds and only have about 4000.  Their population is 210,000,0000.  Texas, California and Pennsylvania have an ICU bed capacity of slightly over 4000 with a combined population of a little over 72,000,000.  These numbers should have a very adverse effect on case fatality rates in these two countries.

Reinfections - probably rare and limited to those 8% to 10% who do not seem to mount an antibody response.  Based on previous experience with coronavirus he does not believe that immunity will be long lasting but he did not define what long lasting means.

TESTING

It has been frequently estimated by leading experts over the last decade, that about one third of the three trillion dollar healthcare expenditures do not contribute to the health of the populace, wasted funds.

The RT-PCR test for Covid-19 is a "point in time test".  A positive result demonstrates infection and infectiousness.  A negative result demonstrates lack of infectiousness but does not reflect an absence of infection.  As discussed above, incubation involves the build up of viral load which will culminate in a positive test at the limits of detection.  Thus, at least two days and as many as 14 days lapse from exposure to a positive test.  A negative test yesterday proves nothing of the results of a test done today or tomorrow.

Currently, Federal legislation mandates that insurance companies pay for all RT-PCR tests without collecting co-pays, if the test is medically indicated.  Medical indications are being ignored, and currently all tests are being paid completely.

Employers and healthcare providers have developed protocols for testing asymptomatic individuals which does not have a positive effect on healthcare.  In the state of New Jersey, ALL surgeries must be preceded by a RT-PCR test.  In most instances, this is accomplished days prior to the surgery which provides zero information to providers taking care of the patient, they assume, therefore, that all patients are positive.  The test has changed nothing about how the patient will be treated and has not improved the health of the population at large.  New Jersey also mandates that all nursing home patients and staff be tested twice a week, this gives no information about the other five days.  In a setting where masks for all are mandated, this adds zero to welfare of the tested.  The CDC has encouraged asymptomatic Covid-19 positive healthcare workers to work, so why the tests? 

The test costs $100.  So far, in the USA, we have spent $2,540,349,800 on RT-PCR tests, this is real money being spent.  Last week we spent $307,347,600 on RT-PCR tests.  A significant  portion of these funds have been wasted because of misguided policies and the ability of any individual, for any reason, to get tested at this time.  This lunacy will have an adverse effects on our economy!

 

NUMBERS - from the week ending 6/20 compared to the week ending 6/13

Alabama - up 6 beds in use, down 167  new cases, up 12,483 tests

California - up 172 beds in use, up 10,623 new cases, up 87,072 tests, the percentage of positive tests to total test increased

Connecticut- up 24 beds in use, down 349 new cases no info on tests

Florida - no info on beds, up 8615 new cases, up 7616 tests, the percentage of of positive tests to new tests increased

Georgia - up 134 beds in use, up 134 new cases, up 18701 tests, the percentage of positive test to total tests went down

Illinois - decreased beds in use, down 1275 new cases, no info on tests

Iowa - decreased beds in use, down 480 new cases, 3793 fewer new tests,  percentage of positive tests to total tests went down

Louisiana - up 30 beds in use, down 701 new cases, 28,693 fewer tests,  percentage of positive tests to total tests way down

Michigan - decreased beds in use, 1077 fewer new cases, no change in percentages positive to total cases

Mississippi - decreased beds in use, 59 fewer new cases, no info on tests

New Jersey - decreased beds in use, 1316 fewer new cases,  up 5908 cases, percentage of positive tests to total tests went down.  

New York - decreased beds in use, 585 fewer new cases, no info on tests

Ohio - fewer beds in use, 742 fewer new cases, no info on tests

Pennsylvania - fewer beds in use, up 218 new cases, up 12,078 tests, percentage of positive tests to total tests went down

South Carolina - up 150 beds in use (28%), up 150 new cases, no info on tests

Texas - up 1000 beds in use (44%), up 8848 new cases, 18,575 more tests, percentage of positive tests to total tests way up

Wisconsin - fewer beds in use, up 98 new cases, no info on tests.

For the states where information was available, the only state that may have had more new cases because of more testing was Georgia.

Alabama had fewer new cases but more testing.  South Carolina and Texas both had significant increases in bed use.

I was pro mask even when the CDC was actively telling people not to wear them. I'm ahead of the curve like that.
