    Hey everyone.  Im still in the hospital.  No ventilator.  No visitors.  Breathing treatments multiple times a day. Chest xrays every other day. Pulse oxygen is 89% with a nonrebreather mask running fu

  • Update  Surgery was a success. Mom has been home since this afternoon. Some pain, but good otherwise and they got the entire tumor.  Thanks all for the well wishes and prayers.

Imagine being so stubborn about something that you'll hold up an entire plane full of people because you don't want to put a piece of fabric over your face during a pandemic. Not only that, but they had to stay in Detroit longer. Cruel and unusual punishment.

Now this pitcher has a heart issue. This is the point. We don’t know all the after-effects. Even if the cases aren’t fatal, careers could be in jeopardy if they are exposed to the virus.

I’m posting from Myrtle beach this week, breathing in the rona. It’s no wonder it’s spreading like crazy down here. They act no differently than pre COVID. No masks, no attempt what so ever to keep any kind of distance.

Glad you're back and you're doing better. I heard about the double negative testing, which is a good thing. I'd like to know how you feel in two weeks to a month as I've heard a lot of people don't fully get over it (stomach problems, etc).

A 29 year old starting guard (for the past 4 years) for last year's Super Bowl winners, the Kansas City Chiefs, has opted out of the 2020 NFL season.  He has graduated from Medical School but did not complete a residency because of his NFL opportunities.

During the past several months he has worked as an orderly in a long term care facility. Playing for the NBA Clippers, was on excused leave from the NBA bubble to attend a funeral.  He got hungry while away and decided to get something to eat at the Magic City Strip Club, a logical choice!  While picking up his food he was also able to buy a lap dance from one of the stirppers, quite an efficient use of his time.  Personal accountability - not so much.

Drunk driving laws, speeding limits, curfews, restraining laws - - all ENFORCEABLE

History is in the making, clearly we are not learning from it.

Covid Testing

In Pennsylvania, at some public health testing centers, individuals are performing (attempting) their own nasopharyngeal swabs.

My wife and I volunteered as "sample handlers" at one of these testing centers in Ardmore on Wednesday.  Individuals were supposed to be guided through the process by a nurse standing inside a trailer some eight feet away from the patient, speaking through a plexiglass window wearing a face mask.  Sample handlers stood outside with the patient.  Of the 24 patients that tested that afternoon, we would say that 20 did not perform the sampling correctly.  We mentioned our concerns to the public health nurse in the trailer and to the nurse guiding the patients.  The nurse guide was quite frank about the fact that from where she was standing, she could not really see how far the patients were inserting the swabs into the nasal passage.  Inadequate/inappropriate sampling is known to be the most common cause of false negatives test results.  It was frustrating for us to deal with the fact that some of these patients who may have had covid, would test negative and go through their daily lives potentially infecting others.  

The turn around times for this testing center was five to eight days.  WORTHLESS as it relates to containing the spread of Covid-19.  These people were not quarantining while they awaited their results.  By the time they would get their results most of them would be non-infectious.

The NIH defines wasted healthcare dollars as money spent that does not contribute to the public health.  This testing center is the textbook case of wasted healthcare dollars and is actually endangering the public welfare being the conduit of false negative tests.

I wrote to the Montgomery County Medical Reserve Coordinator who recruited us to volunteer, the Governor, and  Dr. Levine (Head of the Pennsylvania Health Department)  

Another example of the lunacy that attends the Country's response to Covid containment.  

If you feel that you need a test for Covid-19, then you SHOULD ABSOLUTELY be quarantining until you get the results.  If the results are positive but you are not really ill, you may return to society after 10 days from the test date,  but the last three days must be asymptomatic.  You do not need further testing.

Children

Two weeks ago there was a report in the lay press  from Texas that claimed an extreme number of positive covid cases in infants and one from Florida that claimed an extremely high percentage of Covid positive tests in children.  I have been unable to find any articles in medical journals addressing these results.  Yesterday, the CDC reaffirmed the very small number of cases in children.

I believe these reports in the lay press were an example of fake news.

I have been openly critical of the missteps made by the FDA.  Dr. Hahn took over the lead at the FDA in January of this year, he stepped into the most significant crisis this country has had in a century.

The FDA has seen a doubling of its normal workload since this crisis has begun, and is tasked with regulating over 20% of the products consumers purchase in this country.  An incident command group to facilitate communication across the departments of the FDA has been formed.  Also, a coronavirus acceleration program has been initiated to prioritize applications on the basis of "available science". (it is important to understand that "available science" is rapidly changing as we learn more information about this virus on an almost daily basis)

Not surprisingly, much of the interview focused on the  vaccine program.  Dr. Hahn discussed quality vaccine manufacturing.  It is not as if the FDA  needs to reinvent the wheel here.  They have years of experience with the manufacturing of the flu vaccine and the Ebola vaccine and the MERS vaccine.  However, this will be the first time in the history of the world that vaccinations will be attempted in well over a billion people worldwide.  Every potential flaw will be greatly magnified because of the scale of the project..  ALL manufacturing sites, regardless of their world location, must be approved by the FDA through a thorough inspection process and there will be no exceptions for newcomers. (if they wish to market vaccines in this Country)

There are six vaccines that have been approved by the FDA  to enter large scale (30,000 subjects per vaccine) clinical trials.  There are four more vaccine candidates that are pending approval for clinical trial.

Dr. Hahn emphasized the independence of the FDA from Operation Warp Speed, and that all approvals will be based on data and science and acknowledged that there is likely to be "wasted" funding for vaccines that have been pre manufactured but do not end up meeting the approval process by the FDA.  He is very comfortable with this because of the need to get a safe and effective product to market.  At our current rate of devastation, a 60 day delay could mean another 60,000 deaths just in this country.

Supposedly plans are being developed for distribution of a safe and effective vaccine; who, how, when and for those who cannot afford to pay.

Dr. Hahn discussed "VACCINE ASSOCIATED ENHANCED RESPIRATORY DISEASE", which is a major concern and has been seen in the past with both MERS and SARS vaccines.  This has occured when patients who were mildly symptomatic received a vaccine, and a very significant worsening of their respiratory condition occurs.  This is an ongoing concern during vaccine development because more than likely, the vaccine will be administered to some volunteers with subclinical disease.  This phenomena has not been evidenced so far but the numbers are so very small at this time.

The FDA has set the floor for approval of effectiveness at 50%  This clinical outcome is how the statisticians determined the number of patients (30,000) that needed to be tested for each vaccine candidate to ensure statistical significance.  Immune response and T cell activation will not be used as clinical endpoints at this time because we do not know the clinical relevance of these responses.

50% effectiveness may be either the reduction of the disease from a potentially fatal disease to one that mimics the common cold or the complete prevention of acquisition of the disease.  If, as time goes on, there is an obvious correlation between clinical outcome and immune response outcomes, then going forward, immune response outcomes may be used as surrogates for endpoints  Immune responses outcomes have been used in the preclinical trials which formed the basis for acceptance to proceed with larger clinical trials,  but will be irrelevant from this point on.

Because a safe and effective vaccine will be potentially administered to over a billion patients who may be Covid free, safety is a huge concern.  Currently volunteers are given an app for their phones with surveys that must be filled out daily.  Even with all six current vaccine candidates, only  90,000 individuals will be given the active vaccine (half will receive placebo), this pales in comparison to the billion who may ultimately receive the vaccine.  A serious side effect that occurs only once in 90,000 doses will rear its ugly head 11,110 times when administered to a billion people.  A very robust post marketing monitoring plan is being developed which may utilize phone apps to rapidly detect side effects that may come to light once millions of doses are administered.

The FDA will be demanding the raw data and will not be permitting summaries of the data.  For the first time, the FDA will be utilizing rolling reviews, so that by the time a clinical trial has been completed the FDA may have already reviewed 80% of the data.

Important to me is EMUA (emergency use authorization) which Dr. Hahn, at least in this interview, seemed to have a dim view of.  This is a process whereby a product is allowed to be used PRIOR to final authorization, which I think is an exceedingly poor idea.

Dr. Hahn seemed acutely aware of the trust issues that exist.  An outside, fully vetted, Advisor Panel has been formed to review the science and data just as the FDA will, to ensure independence of process.  Dr. Hahn accepts his leadership role and tried to assure the audience that his only consideration will be the data and the science.

People need to embrace the idea that if an activity cannot be accomplished while distancing or wearing a mask it shouldn't be done.

Nobody deserves to die, but those trying to be sympathetic about him are fing idiots.  It's like saying we wouldn't all laugh at some moron recording himself playing on the interstate bragging how well he dodges traffic and then gets hit by a car.
