August 16, 20205 yr I just found out that my cousin, who has leukemia, got COVID-19 and survived. My other cousin's husband (her sister's husband) and their daughter also got it, albeit much milder. The sisters live down the street from one another and visit frequently, so I'm guessing they somehow shared it amongst themselves. I'm a little peeved that none of them told extended family that she was battling COVID-19, but of course I'm happy she survived it. It was a bad case complicated by her leukemia, but she defied the odds (the majority - 80% or higher - of persons who have her form of leukemia that were hospitalized with COVID-19 died). She's home and has tested negative for a few weeks now.

If enough people trying to follow the advice of medical scientists feet the adverse effects on their health and welfare caused by bad behavior perpetuating the virus they might get serious with the miscreants who are making a mockery of the realities of Covid-19. Until these individuals are dealt with sternly, the future is bleak. These despicable people are the greatest threat to health and welfare of our country. THE VIRUS - it has become clear that viable virus in the body is short lived. (less than 10 days) Evidence continues to grow suggesting an autoimmune response to the virus, put into motion, that can last an extended period of time. Patients who were asymptomatic or minimally ill, may actually have significant effects that do not become evident for weeks or months and we are still in the discovery phase. VACCINATION To the human body, a virus is a recognized as a foreign body often referred to an an antigen. This antigen will initiate a response induced by B cells. A vaccine is a surrogate viral antigen. There is a fast acting primary response and a slower, longer lasting recall response. During the primary response, antigen (vaccine or virus) interacts with naive B cells and gives rise to short lived plasma cells and Germinal Cell B cells. The short lived plasma cells will respond with antibody production, typically IgM, which is neither high affinity nor long lasting. The germinal B cells (once they are produced) will interact with the antigen again in the context of follicular T cells in the lymphoid tissue of the body and give rise to longer lasting plasma cells and memory B cells. A recall response occurs with re-exposure to the antigen. In the case of vaccination, the first exposure is the vaccine, the subsequent exposure would be the virus. Memory B cells, on re-exposure will rapidly produce high affinity antibody, typically IgG. Long lived plasma cells living in the bone marrow are the source of the antibody that we are measuring in the periphery (blood stream) when we are attempting to determine who has been exposed to the virus. There are referred to as neutralizing antibodies. Currently there are two process targets that are keys to vaccine strategy. The first process target is the attachment of the virus to a host cell via the "spike protein". (inhibition of attachment) The second process target is termed "fusion" the process of the virus gaining entry into the interior of the host cell after attachment. (inhibition of viral entry into the host cell) A 1990 study looking at seasonal coronavirus infections showed that by one year antibody levels had waned. Volunteers were inoculated at one year after initial infection. Six of the nine volunteers became infected but only one became symptomatic. In 2011 a study of 26 volunteers who had survived SARS-1, by three years there was no measurable antibody, but what level of antibody would be protective along the degradation curve was unknown. In a study of MERS infections in South Korea, those with only mild disease mounted just a short lived antibody response. Effect of an Inactivated Vaccine Against SARS-CoV-2 on Safety and Immunogenicity Outcomes: Interim Analysis of two Randomized Controlled Studies S. Xia, MD JAMA 8-13-20 From China comes a report of an inactivated vaccine trial ( measles type vaccine). Healthy participants between the ages of 18 - 59 were selected. 96 healthy volunteers were split into four groups of 24, with three different doses and one placebo group. Vaccination was performed at 0, 28 and 56 days All three groups demonstrated an immunologic response with a relatively low percentage of mild side effects. A phase two study was performed with 224 volunteers with the middle dosage of 5 mcg at 0 and 14 days. Again there was a 100% immunologic response with mild side effects, mostly pain at the injection site in a relatively small percentage of patients. The side effects and percentages as reported in this study were lower and milder than in the previous vaccine trials presented to date. This vaccine is now entering into large phase 3 trials which will investigate efficacy and safety. We will need more than one vaccine to treat 7.8 Billion people worldwide. Prevalence of SARS-CoV-2 Infection Among Health Care Workers in a Tertiary Care Hospital A. Jeremias, MD. JAMA 8-11-20 1699 Healthcare workers were tested at this hospital situated in Long Island. 9.6% of these frontline workers exhibited antibodies suggestive of previous Covid-19 infection. On Long Island, population investigations have demonstrated a16.7% presence of antibodies suggestive of prior Covid-19 infection. Despite the fact that the health care workers were exposed to a much higher incidence of Covid-19 patients than the public, and the fact that physical distancing was not possible frequently, their infection rate was significantly less. This suggests that personal protective equipment was effective. A similar study was performed in a New York City Healthcare System. Of the 46,117 tested 13.7% had antibodies suggestive of previous Covid-19 infection. In New York City the prevalence of antibody suggestive of previous Covid-19 infection is 21.2%. Vaccine Refusal and Measles Outbreaks in the US V. Phadke, MD JAMA 8-14-20 Since 2015 there have been eight measles outbreaks in the US. Measles vaccination is accomplished in greater than 90% of the population. These outbreaks included 1176 infected individuals. It is thought that 64% of these cases were initiated by foreign travel of unvaccinated US citizens to areas where measles remains endemic. The travelers returned to the US and infected others who were not vaccinated. The idea of herd immunity seems to be put into doubt by these studies. It is highly unlikely that we will reach 90% vaccination rates with a SARS-CoV-2 vaccine leaving many in the US susceptible to continued Covid-19 infection. LOW SENSITIVITY TESTS v. HIGH SENSITIVITY TESTS In the last several days there has been much coverage of the inexpensive, rapid, low sensitivity tests which the FDA, in their infinite wisdom, has granted Emergency Use Authorization. This means that the companies marketing these tests have not performed the normal validations required under existing guidelines. People defending the use of this test are looking at theoretic models that suggest that the low sensitivity is overcome if testing is performed during high levels of viremia. This would require testing daily for three to five days and again this is only THEORETIC. It appears that these tests are anywhere from six percent to ten percent less sensitive than the gold stand PCR test. This means six to ten people out of every 100 people who actually carry the disease will believe that that there are free of disease. They will not isolate and they will unknowingly infect others. WHAT A TERRIBLE IDEA!!
