  • Update  Surgery was a success. Mom has been home since this afternoon. Some pain, but good otherwise and they got the entire tumor.  Thanks all for the well wishes and prayers.

We go back to normal when Bill Gates is either out of the picture or he gets is wish of vaccinating the entire world, until either of those that happen they'll continue the facade.

BTW, his vaccination is all about population reduction so expect the lockdown to ramp up in wave two, nobody will see the light of day until a year or longer from now, they're already talking about the dark winter.

Estimates of Excess Mortality During the Covid-19 Outbreak in New York City March 11,2020 to May 2, 2020
Weekly Morbidity and Mortality Report, 3/11/20
 
On 3/11/19 the first Covid-19 associated fatality occurred in New York City, the same day the World Health Organization declared Covid-19 a pandemic.  On 3/17/20 the New York City Department of Health and Mental Hygiene (DOHMH) initiated a daily match between all deaths reported to the DOHMH electronic vital registry and laboratory Covid-19.  Death for which Covid-19, SARS-CoV-2 or an equivalent term is listed on the death certificate as an immediate, underlying, or contributing cause of death, but that do not have a laboratory confirmed Covid-19 test, are classified as probable Covid-19 associated deaths.
 
As of May 2, 2020,  a total of 13,831 laboratory confirmed Covid-19 associated deaths and 5048 possible Covid-19 associated deaths were recorded in New York City.  Counting only confirmed or probably Covid-19 associated deaths however likely underestimate the number of deaths attributable to the pandemic.  Confirmed and probable cases do not count persons with SARS-CoV-2 infection who did not access diagnostic testing, tested falsely negative or became infected after testing, died outside a health care setting or for whom Covid-19 was not suspected by a health care provider as the cause of death. (the first documented Covid-19 death in the US, an otherwise health 35 year old, fits into this last category, diagnosis was made post mortem)
 
The objective of the report was to estimate excess deaths that occured in New York City, in the setting of widespread community transmission of SARS-CoV-2.  Excess deaths are the number of deaths above expected seasonal baseline.  Estimates of all cause excess deaths is used as a non-specific measure of the severity or impact of pandemics and public health emergencies..
 
DOHMH has developed an electronic vital statistics reporting system that provides a near complete count of all deaths that occur in New York City.  A seasonal periodic regression model as is routinely conducted for monitoring the impact of seasonal influenza was used to estimate excess deaths in New York City.
 
Excess deaths were determined for the period of 3/11/20 to 5/2/2020.  During this period a total of 32,107 deaths were reported to the DOHMH:  of these deaths, 24,172 (95% confidence limits = 22,980 - 25,364) were found to be in excess of seasonal expected base line.  Included in the 24,172 deaths were 13,831 (57%) laboratory confirmed Covid-19 deaths and 5085 (21%) probable Covid-19 associated deaths, leaving 5283 (22%) excess deaths that were not identified as either laboratory confirmed or possible Covid-19 associated deaths.
 
The 5293 excess deaths not identified as confirmed or probable Covid-19 associated deaths might have been directly or indirectly attributable to the pandemic.
 
Covid-19 associated mortality is higher in persons with underlying health issues such as hypertension, diabetes and obesity and deaths in these patients with underlying health conditions might not be recognized as being directly attributable to Covid-19.  Social distancing practices, the demand on hospitals and healthcare providers and public fear related to Covid-19 has lead to delays (dread risk) in seeking or obtaining life saving care.
 
Monitoring of all case deaths and estimating excess mortality during the pandemic provides a more sensitive measure of the total number of deaths than would be recorded by counting laboratory-confirmed or probable Covid-19 associated deaths.
 
Tracking excess mortality is important to understanding the contributions to the death rate from Covid-19 disease and the lack of availability of care for non-Covid-19 conditions.
 
Contrary to the opinions of many, who felt that the pandemic was overblown, that the numbers were being padded, this report suggests that the direct and indirect consequences on fatalities by the virus have been underestimated.

Unfortunately bio-weapons do exist and if their scare tactics don't work they very well could release one making those death numbers a reality but as of now, the death numbers are in line with the flu season.

I just re-watched Contagion the other night on HBO - it's crazy how much of what takes place in that movie is what we're experiencing right now.  The terminology they use is identical, people are abusing their power to aid loves ones, there's a dude pushing conspiracies and fake cures, etc.  Bad thing about the last point is it's our own damn President pushing the fake cures and conspiracies :lol: :wacko:

With the exception of major urban hubs currently getting hit hard, I think we should reopen everything and many places should have done so weeks ago.  But people should also be encouraged as much as possible to wear masks and maintain social distancing.  If everything is open WITH people doing these things and the uptick is sustainable to the health care system, THEN you can talk about the next step where people are normal again, out and about without any restrictions.

 

But I don't think normal is really going to happen, at least for urban areas, until this is completely gone, post vaccine.

It's a **** situation but what's the alternative for skilled nursing facilities taking these patients though? Unless the family is willing and able to take care of them, or willing to pay for private care, it's really either make SNFs take them or keep them in the hospital forever.  A good friend of mine is an NP/manager at a SNF in Philadelphia and they essentially are putting all the COVID patients in the same building.  It's a Fing nightmare but again what are the alternatives?

As for the re-opening question, we need to recognize that one size fits all isn't working and should be abandoned. The NYT piece a couple weeks ago was right -- we don't all need to live by NYC's rules. NY, NJ and CT have almost 50% of the deaths in the US, and frankly NYC will continue to be a petri dish cesspool until they close the Subway. I would recommend the following:

1. Begin re-opening most of the country and begin relaxing social distancing guidelines as appropriate

2. Restrict travel to and from the NYC area

3. If a new outbreak occurs -- restrict travel and try to contain the outbreak in that area. Now, this requires much more testing capacity in order to quickly identify an outbreak and contain it before it spreads. This is where our failure on testing really bites us in the arse. But, if we get the testing capacity, if there is a breakout in Philly, then shut down travel in and out of Philly for a bit.

4. Be honest about the true reality -- most people are eventually going to get COVID. We need them to get it to hopefully develop herd immunity. We flattened the curve and didn't overwhelm the hospital system at the start -- sadly, we haven't spent the last 2 months building up supplies needed.

This is what we should be doing, but the morons in charge wasted 2 months and didn't ramp up testing, PPE, ventilators, etc. Stop acting like we won and sending ventilators overseas -- excess capacity isn't a bad thing and is what we need to re-open.

I think the shelved CDC protocols were pretty a pretty solid set of guidelines.  Set out real specific goals for reopening with cases/ICU capacity/hospital PPE and staffing/testing percentages, but also emphasizes local approach and the need for increased testing/tracing.  Pretty similar to what you're plugging (minus the NYC specific stuff).
