His handling of this will cost him a 2nd term and his sanity.   He seems to be losing it mentally.  But it also cost so many lives and so many jobs.  We are all going to pay.

but i wouldnt say any other president would have got it right. I have no faith in anyone in DC and they are all equally capable of f’ng up a pandemic situation.

can you imagine how miserable a person trumps going to be when he loses and everyone stops following his tweets and talking about him in the news?   Its going to be pathetic.

meh. I think there are presidents who can be effective in a crisis.

depending on the crisis, that means some combination of:

  1. Understanding the issue, and also understanding the limits of your personal ability to do anything - your job is to lead.
  2. Ability to organize a response, or enable those who can under you.
  3. Working with your messaging team to craft a coherent speech to deliver to the nation, communicating in a digestable way the plan, calming fears by demonstrating that there is a plan, and that plan can be executed.
  4. Also communicating the reality of the situation, and not sugar coating pains and sacrifices that are going to be asked of the nation.

This is just the first part. Then comes the hard part of actually following through with the plan, adjusting as new information comes to light, while keeping an open dialog with the nation.

There are many presidents who would be capable, either through being a strong communicator and acting largely as a figurehead but also as the human touchpoint that the nation needs, or through strong executive ability to manage and organize and demonstrating that leadership by example.

Trump is none of these things. And it's worse than that, because his willingness to cultivate a sense of denial among "his" followers, and continuing need to take zero responsibility and blame everybody else except himself.

SPOILER ALERT: With the current administration, it's not possible.

we could literally turn back the clock to January, with full knowledge of everything that has happened, and this administration would STILL ef it up.

This whole discussion on hydroxychloroquine is so frustrating and annoying.

1) It is not a benign drug.  There are side effects which can be significant.

2) It has yet to be proven that it is effective against COVID-19 (and unfortunately preliminary results of clinical trials aren't looking promising for its use in combating COVID-19).  Anecdotal evidence and case reports don't show causation, they show correlation, and should NOT be used as clinical guidance.

3) Until it has been proven that it is actually effective against COVID-19, doctors should not be prescribing hydroxychloroquine for outpatient purposes, and it definitely shouldn't be used as a prophylactic.

Daily update from the Doc

Good evening,
REOPENING

Numerous stories and pictures are illustrating the scenes of reopening.  As I have suggested before, the outcome will be the consequence of balance between those choosing to live safely and those who choose otherwise.  This, I believe will be dynamic, influenced by the numbers, as they tell their story.  Something will be done when it must be done.  The voices that people hear in contemplation may be more influential than those heard in public.  Time will tell.

VACCINE

Planning for a Covid-19 Vaccination Program
JAMA Network, May 18, 2020

This article can be obtained free online by the general public

A long term solution to the Covid-19 Pandemic will hopefully be a globally implemented, safe vaccination program with broad clinical efficacy.

There are significant barriers and impediments to such an endeavor.

1.  Logistics - beyond the development of an effective vaccine, there is scaling to production (a monumental task) and administration to many millions of people.  In this Country, many states have local Medical Reserve Corp volunteers who have trained frequently in the process of mass administration of medications.  These groups will be of great assistance in helping to inoculate the 330,000,000 people of our country.

2.  Vaccine hesitancy - this will be a major barrier to convincing the populace of the need to vaccinate.  Preparing the minds of the people will be crucial to the success of such a program.  We have many months to prepare for this and we should be starting now.  We should enlist religious leaders, cultural leaders, political leaders (if they can behave themselves), entertainers, sports figures, and respected individuals to help encourage people to enroll in a vaccination program.  Total transparency as to the positives and negatives of vaccination goes without saying.

Recent surveys suggest that 75% of the population would vaccinate if a Covid-19 vaccine were available but only 30% would want to receive the vaccine soon after it became available.

There may be individuals who are ineligible for the vaccine due possibly to age, immunologic status or other pre-existing conditions.

Hesitancy hinges on the perception of necessity.  There are many individuals who have historically questioned the underlying principle of "must vaccinate."  The behavior that has recently been evident in the public does not reassure that this will easily be overcome.  A willingness to wear masks and maintain physical distancing has not been easily achieved.  There has been a palpable indifference to the fatalities caused by  Covid-19,  presumably  related to the fact that the most severely impacted are elderly with serious coexisting medical conditions. There was a time when the vulnerable were most carefully protected.  I have frequently heard comments related to case mortality in Nursing Homes, such as, "if you eliminate the nursing home deaths in our County, we would only have....." as if this population is inconsequential.

The reluctance of society to accept minor inconveniences such as physical distancing and wearing masks does not bode well for a broad based vaccination program which may be viewed as highly inconvenient.

Then are those with religious convictions which would preclude them from accepting vaccination.(as some religious groups refuse blood transfusion in the face of death)  The mistrust of some with the medical system will also need to be overcome.

There are the miscreants who crawl from under rocks to produce fake news and stories to confuse and misguide the public and for whatever other demented notions drive these individuals.  I would be surprised if many of you have not also seen this nonsense making the rounds.   There are already stories questioning the safety of a vaccine that does not yet exist.   Finally there are those who are concerned about the violation of their civil rights and feel that mandating vaccination is tyrannical.

Much of the authors concern over inclusion into a vaccination program seemed to revolve around the concept of "herd immunity"  If a vaccine were 100% effective, we would need to vaccinate at least 70% of the population of the United States to achieve "herd immunity".

I would hope that all individuals who could be vaccinated would be vaccinated and I would do my best to help reach such a goal, but I am less concerned with achieving "herd immunity".  We are attempting to protect a small, theoretical group of people, who might not be able to accept vaccination for medical reasons,(these people will have to continue to wear masks and distance physically when possible) from unvaccinated individuals.  The equation changes for me when it comes to vaccination.   It is not like refusing to wear a mask or distance physically, which places others at risk by the reckless behavior.  The individual who refuses vaccination hurts primarily himself.  Quite frankly, I have grown weary of hearing about civil liberties and tyranny.  If one chooses to jump without a chute, be my guest, enjoy the trip down!

Well by that logic, damage is done and we can fully open now since it's in the country.

Local governors are best to handle their localities needs. Federal government should be there to supply aid. NYC received everything it requested. Testing is currently at a level higher than people actually use in most parts of the country.

Again, Fauci wasn't calling for a lockdown or to change behavior in late February. While I do wish we did a better job, I don't see where anyone was calling for locking down sooner.

You are very quick to forget the dismantling of the pandemic response team and playbook, all in the name of government waste.

Yes. You need to seek treatment at Tula Regional Clinical Hospital in Russia. It's in the article.

They also ran some comments from locals:

Quote

"At least someone has a sense of humor in this gloomy, gloomy reality,” said Sergey Ratnikov.

Marina Astakhova added, "Well done, she raised the mood of the patients.”

I am sure she raised something.

New outbreak in Northeast China looks different from Wuhan.

There is still time to apply the tactics used at the White House to the entire nation. It is THE ONLY solution other than the current one, let people die.

1. Test

2. Quarantine the sick

3. Contact trace

4. Test

It sucks a little that the people my company furloughed are making more than those of us still working. I have also heard of people with PT jobs receiving partial UE along with the $600/wk. It’s a good thing the FED has a money tree. These programs are going to hurt more businesses than it helps, especially the small ones. And we wonder why the big companies keep getting bigger.

So... my wife has a part time job at a credit union making pennies and I keep trying to convince her to quit. I've been at it ever since she had the job but she just likes talking to people.

In the past week she's caught on to the idea of quitting and collecting unemployment because:

1. She doesn't have the necessary hours in the day to teach our youngest.

2. She'd make way more collecting unemployment than what she makes working.

I know quite a few people still working that are making less than people collecting unemployment

I know a few people who were furloughed, then recently went back to work. They were banking more on unemployment than from their jobs. It's a pretty common theme. The problem is unemployment is only temporary. Government can vote to extend benefits, but they may decide not to either.

If she quits without good cause (proof her boss molested her) then she’s not eligible for unemployment.

this. my employer extended medical coverage from the usual 30 days to 60 days when this all started...and have since extended it to 90 days. but that's going to eventually end.
