Hydroxychloroquine, chloroquine, 95,000 dead and climbing, 15 million RT-PCR tests and counting (do you get a discount when you perform this many?), 1,600,000 infected and climbing, millions and millions out of work, convalescent plasma, billions spent on healthcare, monoclonal antibodies, remdesivir...... instead of surgical masks for all.

 

Sadly, this should have been prevented, could have been prevented, can be prevented, but the carnage continues.  Missing in action are leaders, willing to lead, leadership has failed to step up to the plate and tell the people what MUST be done. Count in this group, the medical "experts" who inexplicably got this wrong.  There are finally some recognized Medical experts informing us that masks worn by all can make this go away.  Sure isn't sexy, and not nearly as interesting as all the scientific projects costing billions of dollars, but never the less effective. 

 

Last week I looked at 17 states; Alabama, California, Connecticut, Florida, Georgia, Illinois, Iowa Louisiana, Maryland, Michigan, Mississippi, New Jersey, New York, Ohio, Pennsylvania, South Carolina and Texas.  Of these 17 states, four;  Florida, Louisiana, Michigan and Texas had more new cases than the week before.

 

This week, from this list, nine states have more cases than last week; Alabama, California, Florida, Georgia, Louisiana, Maryland, Mississippi, New Jersey and Ohio.  South Carolina had the largest increase in cases at 64%.  Michigan and Texas came off the list, Michigan just barely.  I spoke to a fried in Atlanta yesterday who told me the State is open now without restriction and very few people are wearing masks.

 

In Georgia, in the last eight days, there have been 5451 new cases of Covid 19, and assume an equal number of asymptomatic patients.  These individuals, 11,000 in number, are walking around spewing a virus  just looking for a host without a mask.  Apparently the citizenry are making that easy. 

 

PEOPLE NEED TO WEAR MASKS (WHEN PHYSICAL DISTANCING IS NOT POSSIBLE) FOR REENTRY TO BE SUCCESSFUL!!

 

 

CAPITALIZING ON IMMUNE RESPONSE

Fresh Frozen Plasma has been around for a very long time in clinical use.  It has been used in the past as a blood volume expander, in the treatment of bleeding problems and other uses.  Its appropriate use has been noted to be relatively safe.  FFP is blood that the cellular elements have been removed from leaving only the plasma or sera.

 

Convalescent Plasma, is plasma taken from people who have had the Covid-19 disease and can potentially be of beneficial use in treating a patient who has not yet had a chance to mount an immune response but is suffering severe consequences of the disease.  It is slightly different from FFP because we are attempting to enhance it for the treatment of Covid-19.  Unfortunately at this time, we do not know if it works as there have been no studies of sufficient power to prove efficacy.  We do not know what is in each unit of Convalescent Plasma because each patient's unit is presumable different from another patient's unit as it relates to the type of antibodies and the quantity of antibody.  This can be related to a different dose of the virus that a patient had, different immune response, different history of coronavirus exposure and different individual reaction.  Each unit is probably actually a mix of humoral and cell mediated immunities.  It is a mix of responses, not a targeted response which complicates the interpretation of how Convalescent Plasma works.  It cannot be standardized and we don't actually know what the precise protective elements are. If Convalescent Plasma is protective it tells us that there is an element of antibody response which can protect us.

 

Very recently, a company has issued a press release reporting a monoclonal antibody that they have developed that has neutralized the SARS-CoV-2 virus in tissue culture.  Certainly encouraging but a significant leap from a tissue culture to the living organism.

 

There are two ways to think about utilizing monoclonal antibodies;

1. as treatment for an ill patient

2.  as prophylaxis against catching the disease

 

The investigation of monoclonal antibody use in the treatment of HIV over the past seven years has been an encouraging strategy to neutralize the virus.  Many companies are manufacturing monoclonal antibodies.  There are variables but we do not know what they are.

This is a targeted response as opposed to plasma where the antibody attaches to an antigen (the virus)  VAriables such as half life and interaction with cellular immunity need to be answered but if they work it shines a light on what the correct target is to neutralize or impair the virus.

 

Vaccines can induce both humoral and cellular immune responses, and in that way they are different from giving Convalescent Plasma or monoclonal antibodies.  There are only two Beta Coronavirus to look at as templates for what to expect. SARS and MERS which both induced immune responses but we have not determined whether they were protective and how long they lasted.

 

The other consideration with vaccines is that of safety.  The intention is to give the vaccine to healthy individuals who would most likely never have gotten the disease or most likely would have gotten only a mild form of the disease.  So beyond efficacy, the safety issue is of paramount importance and will take some time to understand.

Pennsylvania - 71,563 (up 1.10%, up from 0.81%)

 

Maryland - 46,319 (up 1.81%, down from 2.4%, 247 fewer new cases than the day before)

 

California - 93.025 (up 2.46%, up from 2.38%)

 

South Carolina - 10,096 (up 2.03%, down from 5.47%, 313 fewer new cases than the day before)

 

Texas - 55,145 (up 2.91%, up from 0.96%)

 

World - 5,380,774 (up 1.89%, down from 1.96% but over 100,000 new cases again)

 

 

Wear masks when you cannot physically distance, while you are waiting for a vaccine that is VERY unlikely to be 100% efficacious. If enough people wear masks we will not need a vaccine.
