Ask why the campaign officials asked him to do this at a political rally less than 2 weeks before the election. Ask why when they vetted his material they didn't realize that this particular joke that's targetting a demographic they're trying to win over might be too risque. He also made an off-the-cuff watermelon joke about a black person he spotted in the crowd. Jokes are jokes at the end of the day, but you also gotta know your audience, and when you're at a rally that is being televised, your audience isn't just who's in the room. That's the reason it made national headlines for all the wrong reasons. These are unforced errors if you truly believe you're ahead in the polls and just need to run out the clock without fumbling the ball.
