Over the past four months, I have talked so much about masks that I can do it no longer. So, not that they aren't important, they are, but I have said all I can say. VACCINES The preliminary data on three more vaccines has been released. All have produced promising antibody responses and have similar side effect profiles as the vaccine that was presented in my 7-15-20 email. All four candidates will be entering clinical trials by the end of this month with data probably by November. Rapid Decay of Anti-SARS-C0V-2 Antibodies in Persons with Covid-19 F. NEJM, 7-21-20 Thirty four participants (30 with PCR confirmed SARS-CoV-2 infection) had blood analyzed by enzyme linked immunoassay to detect antibody to SARS-CoV-2 anti-receptor-binding domain IgG. Most of the patients had mild disease. The mean age was 43, there were 20 females and 14 males. Thirty one of the 34 had two serial measurements of IgG levels, three, had three levels performed. The first measurement was obtained at a mean of 37 days after symptoms, the second level at a mean of 86 days after the onset of symptoms. Based on the two levels a 1/2 life of 36 days (26-60 days range) was determined. This antibody degradation is quicker than that of SARS-C0V-1. We don't fully understand the nature of the protective effect of antibodies or specifically which antibodies are protective and at what level. Five half lives would represent 95% degradation. It would appear that 300 days, based on this data, is the outer limit of antibody level. Another study (Clinical and Immunologic Assessment of Asymptomatic SARS-CoV-2 Infection, Nat Med 6-18-20, Quan-Xin Lang, MD) has reported similar findings. Based on these reports it does not seem that antibody protection will be long lasting for SARS-CoV-2. By comparison Varicella Zoster antibodies have a 1/2 life of 50 years, Measles antibody 1/2 life is estimated to be 200 years. There are increasing numbers of reports of "second" infections, (which have not been proved by culture of viable virus). This is concerning however, given the fact that immunity may last only three to four months. This should be stimulating Public Health Officials to come up with a plan for vaccination (or other therapies such as monoclonals) that would be effective given the forgoing constraints. Public Health Officials have not been particularly stellar in planning thus far. School - - Opening schools in low prevalence areas such as New York, New Jersey, Connecticut, Pennsylvania and other low prevalence areas should be accomplished, with precise plans for tracking and testing so that we might better understand transmission from younger people to other young people and to adults. Dr. Topol called it "CRAZY" to contemplate opening schools in states such as Florida, Georgia, Texas, California and other high prevalence states. Both Dr. Bauchner and Dr. Topol are very concerned about the complications of school openings on the greater population along with the influence of the Flu come the fall. They have been successfully used in non-human primates both as prophylaxis and therapy. With monoclonals, there is no concern for antibody enhancement as there is with vaccines. Dr. Topol was concerned about scalability, but in a presentation I watched by a research group working on monoclonals, they did not feel that scalability would be a problem and even stated that manufacturing of large numbers of doses would be practical. There are several monoclonal candidates already in clinical trials and these therapies may be available by October. This would be particularly useful for the at risk populations and healthcare workers. Japan had very relative little testing. They were criticized early on for their lack of testing. But they've contained the virus excellently, because they have a population that has wearing masks ingrained in their culture. During normal times, it's customary for Japanese citizens to wear masks if they have a cold so they don't spread their germs to each other. So they had high, early mask compliance during this pandemic. Basically adopted the mindset of, "assume you have the virus when you leave your front door"

On March 19th, they stopped most foreign travel into their country, and any returning citizens were required to quarantine for 2 weeks.
