The Covid-19 Pandemic may be viewed as a dread risk event. Certainly a low probability event, resulting in the deaths of many, generating extensive media coverage. So far, the horrifying images have not been shown to the public. Covid-19 is generating risk avoidance which may result in worse outcomes, or at least undesirable outcomes which could be avoided were it not for the avoidance behavior. RoMed Health System in Rosenheim Germany is experiencing far fewer non-covid admissions to their Emergency Departments than prior to the pandemic. Public health officials there fear that patients are avoiding needed care for fear of contracting the disease. Many Hospital Systems in this Country are experiencing fewer follow up visits for chronic diseases such as Congestive Heart Failure, Diabetes, and Chronic Obstructive Pulmonary Disease. The very conditions that place patients at increased risk from covid infections. Orthopedists, dermatologists, opthamologist and other specialties are seeing far fewer visits than prior to the pandemic. The decrease in these needed medical follow ups is dangerous for the patients and will not offset the tremendous costs that have been generated by this pandemic. The fear of this disease has also resulted in costly, irrational behavior in treating and diagnosing the disease and studying the antibody response to it. (costly treatments that were utilized with no evidence to back efficacy because they could and the climate was right for such activity, all in the name of COMPASSION) Testing, without a plan which will improve public welfare is a waste of precious resources. Unfortunately, the extensive media coverage of this activity only serves to spur the interest. Some was the result of unscrupulous vendors pedaling their useless tests, while the FDA relented to the public demand that is not evidence based. The FDA is partly to blame for this fiasco secondary to their own slow start at the beginning of the pandemic and their desire to get out of the limelight now. The bills will come due in the not too distant future. If commercial insurance premiums increase by 20% to 40% (or more) next year, how many people will drop their coverage or opt for only catastrophic coverage? Needed routine care and preventive medicine will fall by the wayside. This risk aversion does not stop in the healthcare sector. Just because Governors sanction the all clear for activities in the public domain does not necessarily mean that patrons will return as before. This will become especially evident if the infection rates rise because irresponsible freedom seekers refuse to behave responsibly. There will be a baseline of covid activity for sometime which will generate enough fear in enough people that a new normal will be realized. Is there a silver lining? Fortunately I believe there is. Much of the strict clamp down on public mobility was driven by the fear of inadequate supplies of PPD, Ventilators and ICU beds. The public willed the behavior of leadership when they no longer wanted to read those conversations or see those photos. (Youtube did its best to spur this with their fake interviews of so called experts) Much of the supply issue has been remedied, and we have seen how to create ICU beds and staff them with creativity and ingenuity. If there is a resurgence we will be able to tolerate higher burdens of disease without the press talking about "rationing care", or photographing hospital staff running around in garbage bags. The one I'm most aware of took weeks after the first positive case (and multiple patient deaths) to think "hey maybe we ought to segregate people who tested positive from the rest of the population" The best and brightest are not running these facilities. They are poorly financed in most cases.
