How many times have they had trouble with the substitutions now on a 3rd down?   1 timeout called and 1 12 men in the huddle.   It's almost like they didn't practice getting on and off the field properly in situations that aren't pre-packaged in a joint practice.  

 

That's 100% on preparation.

It still has to even when he's out of the pocket. But it was close enough that the refs weren't gonna pull the flag out.
