It is meant so that the plate or whatever you’re attaching it to is between the "arms” or lugs of the shackle so it can rotate freely about the pin. The pin is threaded on the end, as is the inside of the hole through the shackle lug on one side so you thread it in. By rotating freely about the pin, you prevent potential for unthreading the pin.

I'm glad you explained it. I kept thinking about it, since I've rigged a bunch of speaker arrays in my life, but everytime I started talking about riggers I felt racist.......

Imp is correct. I work with these at my job sometimes, though not as big as the one in the previous pic, but the principle is the same no matter. This is how it should look no matter how it’s used:
