HIPAA just means they can’t disclose your personal health information. It doesn’t mean they can’t ask you for it. You can’t just say "HIPAA” when someone asks for you health background. All that questionnaire does when you fill it out at the pharmacy is cover their ass if you die in their chair. It’s a waiver more than a health background.
