HIPAA just means they can’t disclose your personal health information. It doesn’t mean they can’t ask you for it. You can’t just say "HIPAA” when someone asks for you health background. All that questionnaire does when you fill it out at the pharmacy is cover their ass if you die in their chair. It’s a waiver more than a health background.

For my booster I went to the community college where I got my first 2. I just gave them my name and vaccine card and I was good to go. Never filled out anything or signed anything.

It's a little more nuanced than this. By asking for and documenting a specific medical condition, whoever is responsible for that piece of paper or digital record with your PHI on it is now governed by HIPAA guidelines and controls. If you wanted to ask someone to disclose their PHI then it needs to be done in a way that doesn't violate very specific rules on how their answer is shared/recorded. So I'd imagine that in some cases, rather than creating a vaccination process where HIPAA guidelines slows everything down, they create an environment where no PHI is disclosed and they can keep the line moving quickly. In other cases they might actually ask and at that point you just have to assume that your PHI is being protected.

You're not taking a drug off-label 2. People asking their doctors for prescriptions for Ivermectin for off label use, or doctors prescribing Ivermectin at their recommendation for off label use are not lying. You just admitted to lying, and while nobody was hurt and likely nothing will happen, it technically may have been a crime.
