The word INFLUENZA was first chronicled by the French in 1414. By the 1700's the usage of the term influenza had become commonplace. The VIRUS causing yellow fever was discovered sometime in the late 1890's to 1901 and was probably the first usage of the word Virus as an entity as we know it today. The SPANISH FLU (the influenza pandemic of 1918, also referred to by at least 10 other descriptions) was so coined because Spain, as a non-combatant in the Great War, freely chronicled the pandemic as it spread across the country. The other countries were reluctant to discuss the ravages of the Flu in their countries for fear of giving the enemy advantage, so little was made public of its existence or severity in those countries. The country of origin of the 1918 Pandemic is debated, but it is quite possible that it first arose in an Army Camp (Camp Funston) in Kansas, in March of 1918. At the time, the United States had a population of about 100 Million and was busily preparing to train and mobilize millions of men for the American Expeditionary Force to join the conflict in Europe. Whether it was spread by the troops or spontaneously arose in other countries is not clear. The springtime flu sickened tens of thousands, but killed very few, generating little concern. It seemed to abate during the summer months, a War Department memo of 9/6/18 did not even list influenza as a possible threat. The virus had apparently mutated to a more lethal form and a few days later, 9/8/18, this became painfully evident. (referred to as the second wave) At Camp Devens, outside of Boston, its unheralded appearance became obvious and by 9/14/18, 500 victims showed up at the base hospital in a single day. By the first week of October, the 2000 bed hospital had 6000 patients. Sometime during that first week of October the hospital recorded 375 deaths in one day. The virus rapidly spread through the United States and the rest of the World. About 25% of all flu victims developed bacterial pneumonia and about 30 - 50% of them died. The illness was thought to be bacterial in origin. The virus was identified sometime between 1927 and 1933. Many were quarantined, visitors were limited, screens were placed between beds, masks were worn, and ventilation of living quarters was encouraged. The medical community tried experimental vaccines, mouth and nasal sprays (Clorox?) emetics, enemas, honey, tea, whiskey and aspirin with little benefit. There is no mention of hydroxychloroquine! Colonel Charles B. Hagadorn, the base commander at Camp Grant was so overwhelmed by the deaths of his men, he committed suicide. With an incubation period of about two days, the virus was stealthily traveling with the troops on the rails and on the ships and to and from camps and home. (the troops took the place of young vacationers and students) The 5000 miles of trenches in Europe, home to millions of troops living in squalor provided a breeding ground and with the frequent replacement of the ill and dead, fresh hosts to infect. Squalor does not seem to be a requirement, just close quarters, as was seen at the bars this summer. The second wave seemed to subside by the end of December but by the end of January 1919, the third wave made its appearance and did not abate until June of 1919. It too was deadly, but by then many million were immune. All in all, more American troops died of the Flu than of battle wounds. In the Country, 675,000 died, worldwide some 25 - 40 million died. The pandemic died out by the summer of 1919, most likely because of few non-immune hosts to infect. Approximately 59,000 American soldiers died of the flu. When possible, they were all buried with military honors but their deaths were not accorded the same honor of those who died in battle. The exceptions were the military nurses who died of the flu. They were honored as heroes and afforded full military honors at burial. In a book commemorating the nation's military war dead with individual portraits, soldiers who died during the war, numbered 21 per page, each deceased nurse received a full page portrait. The exact number of people infected is hard to calculate. There was probably an asymptomatic or minimally symptomatic presentation but no way to prove it, and records, such as they are, are 100 years old. Shortly after the armistice, an AMERICAN MEDICINE editorial expressed dismay at the extraordinary indifference on the part of the public regarding the influenza epidemic. By that time, the equivalent of over 1 Million deaths had occurred , by current day population. The writers hoped to arouse the public to a livelier interest in its own welfare. If the epidemic had ended in 1920, an election year, I wonder if the authors would have felt the need to write such an editorial. Epidemics have often generated reforms, smallpox inoculations during the Revolutionary War. Typhus, cholera and typhoid during the Crimean War led to the professionalization of nursing and sanitation. The influenza epidemic of 1918-1919 inspired little in the way of progress. For both the military and civilian medical profession it was a grim reminder of their failure. The fact that there has been over 200,000 deaths in this country, although multifactorial, is more a reflection of an indifferent public in denial, rather than a failed medical system. Covid has resulted in a quantum leap in therapeutics and technologic advances reminiscent of the NASA program and its spin offs, which should leave us better prepared to deal with future viral outbreaks before they become pandemics. LONG TERM HEALTH CONSEQUENCES OF COVID-19 Carlos del Rio, MD JAMA 10-5-20 The clinical spectrum of post infection health consequences is not fully appreciated but surely exists and as we learn more, will be better delineated. Definitionally, Post Acute Covid-19 is the persistence of symptoms beyond three weeks of infection, chronic Covid-19 beyond 12 weeks. Post acute states are common after many severe illnesses, not so much for chronic states. There are a large percentage of outpatient symptomatic Covid patients who have not returned to baseline health two or more weeks after the initial diagnosis. Cardiac - In a German study of 100 recovered hospitalized patients, 60% evidenced signs of inflammation on Cardiac Resonance Imaging 71 days after initial diagnosis. Pulmonary - a significant proportion of of patients three months after discharge have abnormal pulmonary symptoms and imaging studies. In a study of 57 patients post discharge 53% had abnormal pulmonary diffusion capacity. Neurologic - In follow up studies specifically looking for neurologic signs it appears that up to 5% of patients queried have evidence of "brain fog" two to three months post infection. This information is concerning but probably will not influence the behavior of the public. Absent a mutation, like that which occurred with the Spanish Flu, dramatically increasing the lethality of the infection, apathy and denial will probably remain and this infection will smolder on, nipping away at the lives and health of many for some time to come. On Tuesday Eli Lilly submitted data to the FDA for EUA of its monoclonal therapeutic drug. They have previously signed an agreement with AMGEN to help manufacture the drug. On Thursday it announced that it is partnering with the Bill & Melinda Gates Foundation to supply drug to low and middle income countries. Commercial manufacturing of the drug will begin in Denmark in April. I believe that there are (or will be) about 100,000 doses available by December. This is great news. NUMBERS Deaths - 214,377 Hospital bed utilization Every state except Georgia experienced an increase in beds utilized. On a whole, the states I track had a 6% increase in beds (1103 beds) Obviously this is not good. Weekly increase in new cases Every state I track had an increase in weekly new cases except Texas. On a whole, new cases went up 19% (27,973 new weekly cases) This will most likely result in a rise in bed utilization again next week. New Jersey is up for the seventh week in a row. Mask on all the time, handwashing, etc. And i still managed to get it. I don't blame Trump. I don't blame anyone. Ish happens. If people go out and choose to gather in large groups and do nothing to protect themselves, that's on them. No one else. I look at it this way. Do you wash your hands after taking a crap?
